Almost 14.5 million cancer survivors are alive in the US today, and that number will grow to almost 19 million by 2024.

Survivorship Care Plans: Background and Significance

Addressing survivorship care is a significant issue because The number of people with a history of cancer in the United States has increased dramatically, from 3 million in 1971 to about 13.7 million today. The increase in cancer survivors is due to both the earlier detection of cancer and advances in cancer treatment.  Cancer survivors are also living longer following a cancer diagnosis and have unique health care needs related to disease and treatment. The 2005 Institute of Medicine (IOM) Report, From Cancer Patient to Cancer Survivor: Lost in Transition, highlighted the unique needs of cancer survivors, including the potential for persistent toxicities, late complications such as osteoporosis or congestive heart failure, financial burden, and psychosocial distress (2). Often, the cancer survivor’s general preventive health can be overlooked as the individual focuses on cancer treatment.

Carevive Survivorship Care Plans

The Carevive Survivorship Care Planning (SCP) platform is a proprietary cloud-based system for the development and delivery of customized cancer survivor information, was built to address major gaps in oncology care by facilitating the efficient provision of clinically integrated supportive care planning, clinical decision support, and patient management strategies. Carevive SCP facilitates evidence-based symptom and distress assessment and management and supportive care across the cancer care continuum, from diagnosis through treatment and into survivorship and/or palliative care. As such, the system is designed to improve the quality, efficiency, and value of survivorship care.

Patient-reported information and clinical data are captured electronically and entered into a proprietary rules engine to generate automated, customized, and trackable care plans comprised of personalized recommendations based on patient-reported symptom data, treatment regimen, current clinical practice guidelines and nationally-established quality metrics. Carevive’s electronic platform enables patients to self-report current concerns such as symptoms and/or emotional distress. These data are then combined with limited clinician-entered disease and treatment data to generate a customized care plan in draft format, for review and editing by the clinician for further customization of recommendations and associated tasks, such as material to read, medical tests and appointments to schedule.

Clinicians review draft care plans automatically generated by the Carevive SCP, and either finalize them without edits or make changes to further customize the care plan prior to final sign off and delivery to the patient. Carevive is intended to address problems important to survivors while minimizing burden for clinicians to provide quality survivorship care.

Carevive SCPs meet IOM recommendations

The IOM recommended that every survivor receive a treatment summary and survivor care plan to improve the currently fragmented state of cancer survivorship care and provide a roadmap and resources for the survivor to maintain optimal health and well-being. Given the variable and complex nature of cancer care, the IOM proposed that an individualized survivorship plan would play a critical role in improving care coordination and outcomes by providing an individualized summary of the cancer treatment, associated toxicities, and a detailed plan for follow-up care and health promotion. The survivorship plan offers the potential to coordinate care that is often fragmented among several providers such as surgeons, medical or gynecologic oncologists and primary care physicians.

After the 2006 IOM report proposed their implementation as a solution for improving coordination and quality of care, SCPs rapidly gained national and international attention. Accrediting bodies such as the CoC quickly adopted as a standard the delivery of SCPs to all patients completing active treatment, despite little available evidence to support their anticipated effects. Although ongoing research continues to examine outcomes of SCP delivery and seeks to link content and processes of SCP delivery to desired outcomes, oncology practices and cancer centers struggle to overcome plentiful barriers to their implementation. Time and resource burden inherent in SCP preparation and delivery remain highest among these barriers, as does a lack of clarity about who is responsible for their preparation and delivery.

Carevive Systems will incorporate your current program and automate your process via HL7 interfaces with your EMR/EHR and Cancer Registry data eliminating manual entry of patient health information. All care plans are customizable to include your center’s resources, network referrals and patient education programs.
