Carevive Survivorship Care Plans
The Carevive Survivorship Care Planning (SCP) platform is a proprietary cloud-based system for the development and delivery of customized cancer survivor information, was built to address major gaps in oncology care by facilitating the efficient provision of clinically integrated supportive care planning, clinical decision support, and patient management strategies. Carevive SCP facilitates evidence-based symptom and distress assessment and management and supportive care across the cancer care continuum, from diagnosis through treatment and into survivorship and/or palliative care. As such, the system is designed to improve the quality, efficiency, and value of survivorship care.
Patient-reported information and clinical data are captured electronically and entered into a proprietary rules engine to generate automated, customized, and trackable care plans comprised of personalized recommendations based on patient-reported symptom data, treatment regimen, current clinical practice guidelines and nationally-established quality metrics. Carevive’s electronic platform enables patients to self-report current concerns such as symptoms and/or emotional distress. These data are then combined with limited clinician-entered disease and treatment data to generate a customized care plan in draft format, for review and editing by the clinician for further customization of recommendations and associated tasks, such as material to read, medical tests and appointments to schedule.
Clinicians review draft care plans automatically generated by the Carevive SCP, and either finalize them without edits or make changes to further customize the care plan prior to final sign off and delivery to the patient. Carevive is intended to address problems important to survivors while minimizing burden for clinicians to provide quality survivorship care.
